 Pursuant to Section 17(f)(2) of the Securities Exchange Act of 1934 and Rule 17f-2 thereunder, as amended, the SEC requires firms to submit fingerprints for all partners, directors, officers and employees, unless they are exempt under those same provisions. 

 This fingerprint card is WET INK only.  Livescan is not set up for this form.

SF-87 Ink Fingerprint Cards. The Form SF87 is an 8″ x 8″ ink fingerprint card used mostly by federal agencies. The purpose of the fingerprint cards is to process a criminal background history report, usually to determine employment eligibility. 

 The SF-87 Fingerprint Card is often used by the military, federal agencies or to complete a federal employment security clearance. The Department of Homeland Security also uses the SF87 “hard card” for new employees.  

NOTE:  FD-258: Fingerprint Chart historically used for contractor fingerprint submissions to DCSA. You can use FD-258.  As of June 2013, contractor fingerprints can also be submitted to DCSA on the SF 87 Fingerprint Chart.

 The Standard Form SF-87A are fingerprint charts submitted to the FBI in order to fulfill that part of the background investigation.  Agencies with a special agreement with OPM and the FBI submit one copy of the SF-87A directly to the FBI .
