Fingerprint Card Scan Capabilities

STEP ONE: LIVE SCAN authorization Form

Livescan Authorization form from UTAH State Agency. 

The Agency you are applying at will supply the Live scan Authorization form that you will send to VREITZ.

*The exception is if you are applying for Utah board of education then get the Billing agency B-CODE and Cactus number from them.

(EXAMPLE: DHS, DACS, EMS, Utah Board of EDU, Child Care License, Real Estate Division, ETC)

 STEP TWO: Obtain a Fingerprint Card (FD-258) to send to VREITZ

Get your fingerprints taken on a FD-258 card from your Authorized Agency in your State. If you are having difficulty obtaining an FD-258 fingerprint card, please click here

Requested demographics to be filled out on the FD258:

  NAM - Name | Signature | AKA - Aliases |DOB - Date of Birth |    Date Fingerprinted | Signature of Official Taking Fingerprints | CTZ - Citizenship | SEX - Sex | RAC - Race | HGT - Height | WGT - Weight | EYES - Eyes | HAI - Hair | POB - Place of Birth |   SOC - Social Security Number| *OCA (if APPLICABLE)

STEP THREE: Signed Consent to VREITZ. 

Please click the button below to print the consent form to submit your information to UTAH BCI. We need your permission to submit your information to the State.

STEP FOUR: PAY and make a copy of the receipt to send to VREITZ

 (The cost covers Fingerprint Scan services only).
